HEALTH GUARANTEE AND CONTRACT
*Shawnee Goldens (English Cream Golden Retriever breeder) guarantees that this puppy has had proper care to date and has been examined by a licensed veterinarian. Puppy has been dewormed and had first set of vaccinations. If within 72 hours of receiving the puppy, he/she is examined by a licensed veterinarian and found to have a life threatening congenital defect, the owner shall immediately notify breeder and provide documentation from the veterinarian describing the puppy's said condition, and shall return the puppy and the AKC registration papers to the breeder within 3 days of the exam. Breeder will then replace the puppy or offer a full refund, minus shipping. This does not include treatable and preventable illness, or parasites, fleas, ticks, mites, coccidia, giardia or any other parasite, as they are common in dogs and puppies. Breeder assumes no responsibility for vet or shipping fees.
*Shawnee Goldens is not responsible if the puppy contracts a communicable disease after 72 hours of leaving breeder's residence.
*Shawnee Goldens guarantees puppy against any congenital defect causing death until 1 year of age. This health guarantee is extended to 2 year, if NuVet supplements are purchased with autoshipping so that breeder can track usage.
*Shawnee Goldens offers a 1 year hip and elbow warranty, of moderate or severely affected hip or elbow dysplasia, extended to 2 years with NuVet autoship supplements. The puppy must have an x-ray done by a licensed veterinarian to confirm diagnosis and must be documented by an offical OFA consultant. If this occurs, the buyer can choose to return the puppy for a full refund (minus shipping, training & vet fees) or keep the puppy and receive a 50 percent refund of purchase price only.
*Shawnee Goldens offers a 1 year heart guarantee, extended to 2 years with NuVet auto ship, against heart murmurs that rate as severe (Grade 5/6 or 6/6. The puppy must have a canine cardiologist perform an echocardiogram to confirm the diagnosis. The Buyer can choose to return the puppy for a full purchase price only refund, or keep the puppy and receive a 50 percent refund.
*Shawnee Goldens reserve the right to a second opinion from our own veterinarian for any medical issue. We reserve the right to DNA tests to determine said puppy is from our lineage in the event of a dispute. Any legal dispute or proceedings must take place in the breeder's county.
*Shawnee Goldens does not guarantee size, color, temperament, championship, training or temperament of your puppy/dog.
Buyer agrees:
1. ________(initial) (A.) To have puppy examined within 72 hours of purchase. If you are unable to reserve an appointment within that time frame, buyer agrees to notify breeder immediately.
(B.)This puppy is not being purchased for resale and will not be sold to any person, pet shop, puppy mill or agent of the same. Any transfer of purchase must be approved by the breeder.
Buyer agrees to provide annual vet exams and proper medical care for this puppy.
(C.) Buyer is to spay or neuter their dog within 12 months of purchase. No litters can be produced from their dog. Puppy can only be registered with AKC (American Kennel Club).
(D.) Buyer agrees to provide adequate housing away from the elements. If the dog is to be housed outside, buyer agrees to provide a fenced in yard or kennel, and to never chain their dog. The buyer will groom, train and care for their dog in a manner consistent with high quality animal husbandry.
(E.) Buyer will return their puppy to Shawnee Goldens if they are unable/unwilling to care for their puppy. No refund is given in this event. Buyer agrees to feed a high quality puppy food with a medium protein of 24-26% and medium fat of 14-18% with a calcium/phosphorous ratio of 1.5/1 until one year of age. At this time, we recommend switching to a high quality large breed dog food.
(F.) Buyer will hold harmless the breeder and acknowledges that Shawnee Goldens and any of their employees or affiliates, shall not be liable for, nor shall any measure of damages include, any indirect, incidental, special, exemplary, punitive or consequential damages or penalties for loss of income, value, profits, savings, or any loss arising out of or relating to it's performance or failure to perform under this Agreement, even if the party against whom liability is sought to be imposed has been advised of the possibility of such damages. In no case is the the Breeder liable for more than the sales price of the puppy (not transportation, veterinary bills or training).
To be covered by this guarantee, the following conditions need to apply:
1.________(initial) (A.) Puppy must be owned by the initial owner shown in this agreement.
(B.) In case of sudden demise of puppy, breeder must receive a written notice of puppy's death from owner's vet, along with a necropsy report indicating cause of death.
(C.) We strongly recommend buyer continue to give puppy 1 Nuvet Plus waffer each day, which is a powerful antioxidant that gives a tremendous boost to the immune system to prevent skin and coat problems, and creates a defense against over fifty major diseases, including hip and elbow dysplasia, and extending the lifespan of your dog. Limitations of Guarantee
________(initial) This guarantee is non-transferrable. The puppy must be in good physical condition. This warranty is null and void if dog has been bred. A licensed veterinarian must do all laboratory verifications. The buyer/owner is responsible for all transportation costs to and from breeder. The breeder is not responsible for any and all veterinarian expenses accumulated on returned or replaced dog or puppy. The breeder is not responsible for any and all dog training expenses accumulated.
THE FOLLOWING WILL VOID ALL GUARANTEES:
1.________(initial) (A.) Allowing your dog to become obese or severely underfed at any time.
(B.) Evidence of neglect or abuse.
(C.) Previous related trauma (broken bones, related injuries, etc.)
(D.) Evidence of strenuous exercise, such as: treadmill, excessive jogging during first year, extensive standing on back of legs or excessive tennis ball chasing and retrieving-especially on slippery floors.
(E.) Giving your dog any flea/tick medication orally. Medicines such as Trifexis, Comfortis and Bravecto have caused serious and even fatal side effects on puppies. The only flea/tick medication you give must be topical unless permission is granted by Shawnee Goldens.
(F.) Feeding a grain-free dog food without taurine supplements. Canine Cardiomyopathy has killed numerous puppies caused by feeding grain free, low taurine food. Warranty is completely null and void if dog develops cardiomyopathy on said food.
